Hello Cultivators!

This survey is designed to identify the support that Cultivators are seeking during this time of
uncertainty in the establishment of the NYS Cannabis Industry. The CANY Cultivation
Committee will develop initiatives meant to address the prioritized needs of our community
based on the collective responses to the survey.

The survey should take less than 10 minutes to complete and is organized as follows:

Questions 1-7: Basic Demographic Information (1 minute – mixed optional & required)
Question 8: Ranking the Issues that Most Affect You (4 minutes – required)
Question 9: Sharing Your Cannabis Story (4 minutes – optional)
